Ban Request Instructions [READ FIRST]

Do not post unless you are directly involved incident that the requester is describing for the ban. Do not put your "two cents" in--this will NOT be tolerated.

If you feel that you have information directly related to the ban, consider PMing the D. Leader involved before making a public post in this forum.

1. When requesting a ban for conduct, please have a demo or screenshot handy for the admin who contacts you. Also make sure to have a screenshot of the person's Steam ID/hash if possible on hand.

2. When requesting a ban for hacking offenses please have a demo and a screenshot of the person's Steam ID/hash ready for the admin. Bans will not be granted based on hacking accusations without a demo. Demos will be viewed by multiple admins to determine if hacks were in fact used.

3. The topic of your post should only contain your alias/in-game name that you are requesting to be banned.
  For example, in PR, my name is "thekMac" and I append my tag "[IAM]" to it.  Thus, I would write: "thekMac" or "[IAM] thekMac" as the thread title in this section.  This purpose of this is to improve the clarity of the posts and avoid frivolous title names.

4. Please write an extremely descriptive post that details the conduct of the player, the rule infraction, the time and date of each offense and the server in which it occurred.
